The requirement was not met as evidenced by: LPA Lujan observation on 08/19/2026 and 08/27/2026. Based on interview with owner Brianna Harvey, new flooring, paint, toilets, new changing table/countertops/storage space were constructed for all rooms. Per Brianna constructed started July 29 and was completed on August 20, 2026.
(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Based on interview with owner Brianna, she reported that children were placed in room 2 during construction which room 2 is under their preschool license from July 29-August 10, 2026. — requirement not met
(e) All individuals subject to a criminal record review...shall prior to working...in a licensed facility...(2)Request a transfer of a criminal record clearance as specified in Section 101170(f) The requirement was not met as evidenced by: On 08/19/2026 at 11:35 a.m., Staff #1 Alicia Moore was present in Room#3. Per LPA Guardian Roster staff are not associated to the above facility.
Reporting Requirements — based on record review facility failed to report the incidents occurred on 12/4/23 and 4/7/23 which poses a potential health and safety risk to children in care.
Outdoor Activity Space — interviews conducted. Staff and children interviewed disclosed that they have observed cat feces in the sandbox. This is a potential risk to the health and safety to the children in care.
Indoor Activity Space — observations and interviews conducted. This is a potential risk to the health and safety of the children in care.
