Any unusual incident or child absence that threatens the physical or emotional health or safety of a child shall be reported to the Department within 24 hours of the occurrence. This is not met as evidenced by: center failed to report an unusual incident that occured on 02-05-2024 stating day care children being locked out of the facility w/ Staff #1
Personal Rights — child #1-3 disclosed that on different occasions when the children were not following directions; Staff #1 would grab the children in a rough manner causing pain because they were not following directions. This is a Type B citation that if not corrected could pose a potential health and safety risk to the children in care.
