Responsibility for Providing Care and Supervision — Per staff interviews, two different preschool aged children were left outdoors without supervision on at least two different occasions. This poses an immediate health, safety or personal rights risk to persons in care.

Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption...prior to employment, residence, or initial presence in the facility. Based on observation and record review, it was found that two (2) staff members did not have the appropriate fingerprint clearance on record before working at the facility. This poses an immediate health, safety, or personal rights risk to children in care.

Responsibility for Providing Care and Supervision — Per staff interviews, staff did not provide appropriate supervision in the classroom, which resulted in a child having possession of sharp/pointy objects (staples) on several occasions. This poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds - The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. During the indoor inspection, LPA observed the area under one sink in the Orange classroom to have water damage with warped boards, discoloration and dark marks on the walls of the cabinet and there was a dark, powdery substance on the bottom of the cabinet under the pipes. LPA observed that there was a musty odor coming from under the sink and into the classroom. — requirement not met

Responsibility for Providing Care and Supervision — Per staff interviews, staff did not provide appropriate supervision during outdoor play, which resulted in a child becoming injured with a fractured arm. This poses a potential health, safety or personal rights risk to persons in care.
