(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not met as evidenced by: Based on LPA observation and interviews, on 3/11/2026 staff did not have visual supervision of C1 and C2 resulting in C2 pushing over a bookshelf on to C1 which poses a potential risk to the health, safety, and personal rights of persons in care.
101223 (a)The licensee shall ensure that each child is accorded the following personal rights:(2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Which was not met as evidenced by: Based on LPA interview, facility did not ensure bookshelfs (31”high x 12”wide x 48”long) were properly secured resulting in one bookshelf falling onto C1 which poses a potential risk to the health, safety, and personal rights of persons in care.
