Notice to parents that site visit reports are available — Based on record review children’s files, both current and newly enrolled, did not contain a signed form LIC9224 which is required after receiving a Type A citation, dated 10/04/2024.
Child Care Center Director Qualifications and Duties — Based on interview and record review, the licensee did not comply with the section cited above as they have been operating without a Director which poses a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on interviews conducted it was corrobored that an aide (S2) was left alone and was providing care and supervision to children. This poses a potential health and safety risk to children in care.
