Personal Rights — Bases on documents received it was noted that the assistant,S2 accidentally scratched child, C1. C1 was scratched on their cheek by while S2 attempted to take a tablet away from C1. C1 received a scratch on their cheek which posed an immediate health, safety or personal rights risk to persons in care.

Personal Rights — S1 admitted C1 bit C2. According to S1, she was sitting on the couch watching the children play. C1 and C2 began to fight over a toy and suddenly C1 quickly bit C2. S1 did not separate the children, preventing C1 from biting C2, which poses a potential health, safety or personal rights risk to persons in care, if not corrected.
Personal Rights — Based on S1 interview statement children are picked up 5 to 10 minutes late sometimes. S2 stated C1 was picked up at 2:30pm, instead of 2:00pm on 2/8/2024. C1 was not left unattended at school, but not picked-up at the agreed upon time which poses a potential health, safety or personal rights risk to persons in care, if not corrected.
Family home — home free of safety hazards — Based on observations, the licensee did not comply with the section cited above in 1 out 1 heater in bedroom #2 was made accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review the licensee did not comply with the section cited above in 3 out of 3 staff members did not have Mandated Reporter Certificate which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review the licensee did not comply with the section cited above in 2 out 3 employees did not have immunizations on file which poses a potential health, safety or personal rights risk to persons in care.
