Responsibility for Providing Care and Supervision — Based on an unusual incident report, staff statement, and camera footage, a child (C1) was left outside in the facility playground for 8 minutes which poses an potential health & safety risk to children in care.
Reporting Requirements — Based on interviews, staff who directly observed or later became aware of the incident on 5/1/24 did not report it to the Department within the next working day. This poses a potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — On 7/26/23, licensee failed to maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for faucet "A" and faucet "J". Water testing results identified with Action Level Exceedance as defined in WD section 101700.3. are not deemed safe to drink. This is a potential health and safety risk to children in care.
