Personnel Requirements — Based on file review and observations, a staff member was found working directly with children without a fingerprint clearance in classroom 2, this poses an immediate health, safety or personal rights risk of children in care.

Responsibility for Providing Care and Supervision — More than 50% of people interviewed disclosed that children’s toileting needs were not being met and that there was supervision concerns due to all the changes in staff.

Personal Rights — Based on LPA interviews conducted with staff, parents, children and third party interviews, it was determined that staff engaged in conduct that included yelling at children and treating them with disrespect. Additionally, one incident of improper physical handling of a child was directly observed. These actions pose a potential health, safety, and personal rights risk to children in care.

Staff-Infant Ratio — Based on LPA's observation and staff interviews, Room 1 was out of ratio with 9 infants and 2 staff which poses a potential risk for the health, safety and/or personal rights risk for the children in care.

Reporting Requirements — On 3/18/2026 an incident occurred where an infant was fed another infant's bottle with breatsmilk, and the incident was not reported to licensing and an Unusual Incident Report LIC 624 was not received. This poses a potential health, safety or personal rights risk to the children in care.

Personal Rights — On 3/18/2026 an incident occurred where an infant was fed another infant's bottle with breatsmilk. Per documentation and staff interviews it was found that the infant was given the wrong bottle. It is unsure if the infant drank the breastmilk. This poses a potential health, safety or personal rights risk to the children in care.

Staff-Infant Ratio — Based on LPA's observations during inspection, 5 infants were left alone with one Fully Qualified teacher. Three infants were observed napping and 2 fully awake in the process of being fed which poses a potential health, safety or personal rights risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on LPA's observation and staff interviews, Room #4 is being used for the infant program (ages 12-18 months) and Room #4 has not been approved by licensing which is a potential health, safety or personal rights risk to children in care.
