Health-Related Services — Based on LPA observation, the licensee did not comply with the section cited above in one out of two children who have IMS needs. C7s medication was not stored with the original box including an unaltered prescription label. C7s medication also expired 10/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on LPA observation and interview with director, the licensee did not comply with the section cited above in that there are several large divots from wear in the soft pour floor covering being covered up with large commercial style rugs. Many of these rugs have curled up edges, causing a tripping hazard. This poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on LPA's review of the children's sign in/out procedure, several children were not signed in or out. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Activities/Napping — Based on LPA observation, one teacher was supervising 37 napping children which poses a potential threat to the health, safety, and personal rights of children in care.
