Child's Records — Based on LPA records review and Director interview, the licensee did not comply with the section cited above. C1 does NOT have LIC627 in file. In addition, C2 does NOT have LIC701 in file and C3 and C4's LIC701 are missing parent signatures and dates which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on LPA observations and staff interviews, the licensee did not comply with the section cited above. LPA observed in preschool classroom - Huggables, children's personal water bottles in a black bin laying on top of one another. In addition, LPA observed extra personal water bottles stored underneath the classroom sink. Water bottles were not stored properly which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on that the facility did not report incident to CCL as stated in LIC809 within 24 hours of the occurance. This poses a potential risk to the health, safety, or personal rights of children.
