Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in that LPA observed three wood panels located under the patio that had sharp nails, that area accessible to children in care. LPA also observed that there is a potential tripping hazard by the water hose located in the backyard, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in that LPA observed that two staff members Staff 4 and Staff 5 did not have their LIC503 completed, which poses/posed a potential health, safety or personal rights risk to persons in care.
