Buildings and Grounds — Based on observation, facility did not comply with the section cited above. During the outdoor inspection, LPA observed the fence that surround the playground has 4 broken plastic slats in different part of the surround fence; one side of the fence has two brokens plastic slat fence, and the other side of the playground fence has two other brokens plastic slat fence. LPA observed that children in care can easily access throughout the broken plastic slate fences to the street which posed a potential health, safety, or personal rights risk to persons in care.
Advertisements and License Number — Based on observation, facility did not comply with the section cited above. During the outdoor inspection, LPA observed a large banner hanging on the playground fence facing the public street sidewalk advertising the daycare facility. The banner had the name of the facility, age of enrollment and facility phone number; the facility number was not on the banner which poses a potential health, safety, or personal rights risk to persons in care
