Family home — home free of safety hazards — Based on observations, the licensee did not comply with the section cited above. Fire extinguisher was manufactured in 2021 and had not been serviced since purchase, this poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observations, the licensee did not comply with the section cited above in 3 out of 3 cribs had foam attached with zip ties, towels draping over the crib or items stored inside a crib used for infant sleep. Children were not sleeping in the cribs at the time of observation. This poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in 7 out of 13 files were missing immunization records or the records had not been updated in over one year. This poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed there were blankets, pillow and toys in the crib and one infant was sleeping there.
Inspection Authority of the Department — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. At time of arrival at 8:35 AM the staff did not granted access to Licensing Program Analysts (LPAs) in the day care.
Notice to parents that site visit reports are available — Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not provide the LIcensening Report to the parents of currently and newly enrolled children. Licensee did not obtained the LIC 9224 for Typa A cited on 11/06/2024.
Family home — home free of safety hazards — Based on inspection of the safety gate which is missing poles on the gate door that separates the family room and the kitchen which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on reviews of two staff files which were missing immunization record the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on file review, Licensee does not have a child's file for C6 which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on Child's record review for C1 which poses a potential health, safety or personal rights risk to persons in care.
