101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This regulation has not been met as evidenced by: A small cockroach was observed crawling out from/and back underneath a small shelving unit in one of the classrooms.
Responsibility for Providing Care and Supervision — Based on interviews, the licensee did not comply with the section cited above because it was determined during transition to the outside playground a child was inadvertently left behind in the classroom unsupervised which posed a potential risk to the health, safety or personal rights of children in care.
Reporting Requirements — Based on interviews and record review the facility did not comply with the section cited above by not notifying Licensing of a child being left alone in a class, and a child bein pulled to the ground by a staff member which poses as a potential risk to health, safety, and personal rights of children in care.
