Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above where a child was observed to be entering a classroom alone without any supervision which poses an immediate health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observation, the licensee did not comply with the section cited above in director nor staff were available to assist LPAs for inspection as director was not present and lead teacher needed to be in ratio which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation, the licensee did not comply with the section cited above there were multiple occassions where Staff #1 was left out of ratio when Staff #2 exited the classroom which poses an immediate health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 3 out of 5 teachers do not have the required mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of the 5 staff files that were reviewed did not have current immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Written directives for drinking water lead testing — Based on record review, the licensee did not comply with the directive above, as three outlet (outlet used for food preparation and drinking) out of five outlets tested with an Action Level Exceedance (ALE). This poses an immediate Health and Safety risk to children in care.
