Staff (S3) does not of proof of Measles on file and Staff (S4) does not have immunizations records( influenza, pertussis, and measles) on file which poses a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on observations, interview and records review, the licensee did not comply with the section cited above. LPA observed two staff (S3 &S6) are teacher aid, present with children in Room 2. Director, Tomomi Kusuyama stated that for Staff (S3), the degree is from Japan and has not been evaluated within the Unites states of America and Staff(S6) is a teacher aid in room 2, which posesa potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in Rooms 1,2,3, and 4 accessible to the children, which poses a potential health, safety or personal rights risk to persons in care. LPA observed a ant killer spray bottle including the cleaning supplies in Room 2 accessible to the children.
