101212(d)(1) below, a report shall be made to the Department... within the Department's next working day... in addition, a written report containing the information specified...(d)(2) below shall be submitted to the Department within seven days... — Based on interview and record review, the facility did not comply with the section cited above. The Director and Acting Director did not report the incident to Department after being informed of the incident which poses a potential health, safety, or personal rights risk to persons in care.
Child Care Center Director Qualifications and Duties — a complaint report, staffs' interviews, and LPA's observation. The former director left on 8/16/2024 and our office did not receive any qualifications document regarding any new director. This is a potential risk to the health and safety of children in care
Preschool Program with Toddler Component — staff interviews, file reviewing, and LPA's observation confirming that C1 from Toddler Option Program have been with preschool children. The licensee failed to abide with this section of Title 22 Regulations. This is a potential risk to the health and safety of children in care.
