Reporting Requirements — Based on interview, record review, the licensee did not comply with the section cited above in the licensee by admission did not notify the department of an unusual incident on 10/23/2023 involving a subject staff and subject child falling while the staff was caring for the child, which placed the child at immediate risk, which poses a potential health, safety or personal rights risk to persons in care.
