The requirement was not met as evidenced by:1 out of 6 children files review and Child #1 (C1) was missing LIC9150 Additional Children in Care form during the children's file review, photos taken.
(1) The licensee shall request the child's..authorized representative to sign and date the bottom portion of the notice form LIC 995A (8/06)... The requirement was not met as evidenced by: 2 out of 6 children files reviewed incomplete. Child #1(C1) & Child #3(C3) file was incomplete missing LIC995A parent rights form, photos taken of children file of C1 and C3.
Family home — home free of safety hazards — Based on LPA Calderon's observation fireplace had a collapsable screen and a metal screen both aren't screened off and children can access fireplace , the licensee did not comply with the section cited above in [1] out of [1] fire place which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on LPA Calderon's observation during record review Staff assistant #2 mandated reporter exp 3/19/25 and Licensee's training is in progress due to glitching website unable to complete, the licensee did not comply with the section cited above in [2] out of [2] staff assistant mandated repoter certficate which poses a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on LPA Calderon's observation during record review staff assistant #1 and #2 did not have LIC9052 Employee Rights in file, the licensee did not comply with the section cited above in [2] out of [2] staff records incomplete which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on oLPA Calderon observation during record review staff assistant #1 and #2 missing immunization records, Staff #1 missing mmr. tdap, tb and Staff #2 missing mmr and tdap record, the licensee did not comply with the section cited above in [2] out of [2] immunization records which poses a potential health, safety or personal rights risk to persons in care.
