Outdoor Activity Space — Based on record review and interviews, the sandboxes were not being inspected daily and keep free of hazardous material when children have stepped in feces, picked up the feces and placed feces in their mouth. This poses an immediate Health, Safety, or Personal Rights risk to persons in care.

Teacher-Child Ratio — Based on observation, the licensee did not comply with the section cited above when Staff 1 was supervising 16 children, which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation and interview, the Licensee did not ensure a teacher child ratio of one teacher and one aide for every 15 children in attendance when on 11/3/23, one teacher and one aide were supervising 17 children, which poses an immediate health and safety risk to children in care

Responsibility for Providing Care and Supervision — Staff reported that when staff noticed the incident, Child #3 (C3) was already biting Child #4 (C4).

Personal Rights — Based on record review , the licensee did not ensure each child was accorded dignity when S2 told the classroom that monsters were in the closet as a means of stopping the children from opening the closet door in the classroom. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Storage Space — Based on interviews and LPAs observation the Center did not ensure each child had an individual storage space for personal belongings when LPA observed children’s clothing and diapers for multiple children in one storage space on January 23, 2025.

Buildings and Grounds — Based on LPA observation and interviews the center was not clean, safe and sanitary as the toys in the classroom had brown dirt marks from use, the potty-training seats left marks on toilets seats and paint was peeling and chipping off the walls in a classroom. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Teacher-Child Ratio — Based on interviews and observations, teachers are responsible for cleanliness of the classroom. In order to complete this task, the teachers are performing housekeeping duties while providing care and supervision. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Personal Rights — Based on record review, the licensee did not ensure each child was accorded dignity when S1 made threatening comments regarding police contact to C1. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Admission Policies — Based on record review, the child care center is a Title 5 program therefore the policies and procedures must comply AB 2806. A NOA was not issued at least 24 hours prior to C1s termination. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above when brown and green weeds, and wood with sharp edges and peeling paint was observed in the outdoor activity space accessible to the children which poses/posed a potential health, safety or personal rights risk to persons in care.
