Staff-Infant Ratio — Based on observation, the licensee did not comply with the section cited above when one teacher and one aide were supervising 9 infants, which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — The staff did not visually observe the incident.

Infant Care Teacher Qualifications and Duties — Based on interviews and record review of Staff #6 (S6) transcripts, S6 does not have at least three units related to the care of infants.Thus, S6 is not a qualified infant teacher which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care General Sanitation — Based on interviews and observations, the staff was not washing, cleaning and sanitizing when the crickets 2 classroom carpet had debris, the mirror had smudges, and the portable sink used for handwashing was not functioning. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on record review and interview , the licensee did not comply with the section cited above when the 15 minute checks were not being documented for the children over the age of one, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in two of five personnel files reviewed were missing a health screening. which poses/posed a potential health, safety or personal rights risk to persons in care.

Modifications to Infant Needs and Services Plan — Based on interview and record review, the licensee did not comply with the section cited above the needs and service plans were being updated every 6 months instead of quarterly, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in two of five child files reviewed were missing the medical assessment. The children have been enrolled for over 30 days. This poses/posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space for Infants — Based on observation and interviews, the licensee did not comply with the section cited above, in classroom 1 children were playing on the floor while other children were sleeping in cribs. Therefore the sleeping area was not physically separate from the activity space. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation, the licensee did not comply with the section cited above in the diaper changing table in classroom 2 was not within an arms reach of a sink which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above, in classroom 1 scissors were observed on the table, in classroom 1, classroom 2, and the kitchen, items labeled "Keep out of reach of children" were located on low shelves behind closed cabinets, which poses/posed a potential health, safety or personal rights risk to persons in care.
