Personal Rights — Based on interviews, the licensee did not ensure that children in care were not exposed to toxic substances based on C4 claim that they were seated at the table while teachers sprayed the table with Lysol which poses a potential Health, Safety, or Personal Rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 4 out of 6 staff did not have the required mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review the licensee did not comply with the section cited above LPA observed staff #1 and staff # 3 were missing their measles which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation the licensee did not comply with the section cited above in that cusioning under two level play structure had multiple indentations which poses a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on record review, the licensee did not ensure all staff had complete files in 1 out of 5 staff were missing immunizations, mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
