Personnel Records — Based on record review, the licensee did not comply with the section cited above in that LPA observed that staff files for Staff#2, and Staff #3 were not complete with immunization, health records. and Manadated Reporter Trainings, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Staff #1 stated that cat feces have been an ongoing issue due to stray cats near the school. LPA’s observations, record review, and interviews confirmed that children have come into contact with cat feces on the playground, indicating that staff did not ensure healthful accommodations. This is a potential risk to the health, safety, and personal rights of children.
Outdoor Activity Space — During the investigation, LPA observed the cement pathway to the playground was raised by tree roots and contained holes, posing a potential health, safety, and personal rights risk to children in care. This is a potential risk to the health, safety, and personal rights of children.
Buildings and Grounds — Based on observations stated in LIC809 report dated 2/10/2025, the licensee did not comply with the section cited above in that LPA observed in Classroom #7 toilet, light fixtures, ceiling tiles need repair and a baby gate for storage area. In Classroom #8 the ceiling tiles need repair, a doorknob spinner or latch for boiler, and baby gate for storage area. In playground, cushioning is needed, storge needs lock, and area next to storage inaccessible to children in care. All which is stated in LIC809 report, which poses/posed a potential health, safety or personal rights risk to persons in care.
