Family home — in-ground pool drowning prevention requirements — Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed the licensee's pool inspection record and LPA Garcia only observed initials 1x per month. Licensee stated she believed it was only monthly instead of daily.
Mandated reporter training — After LPA Garcia conducted Care Tools finalization, LPA Garcia noticed that the report from last year stated licensee mandated reporter is valid until 9/26. LPA Garcia could not remove this type b violation from the care tools.
Family home — preventive health training, including pediatric CPR and first aid — Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA Garcia observed a certification from the licensee which was from National CPR Foundation which is not an EMSA approved training provider.
Family home — in-ground pool drowning prevention requirements — Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee was not aware of regulation that took into effect in 2025. (I) An alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water.(B) A rescue pole with a body hook and a minimum fixed length of 12 feet. (c) Life Ring that is certified by USCG.

LPA Garcia observed that in the inspection prior it was discussed that staff needed to show proof of immunization.

Licensee stated that the child's parents did not want to vaccinate their child and LPA Garcia observed a hand written note from parents in the child's file that states "have refused to vaccinate my son..due to personal and private beliefs..."
