Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that LPA observed Child 2 napping in the play yard while wearing a bib which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that LPA observed Child 2 was napping in the play yard with a pacifier with a string attachment which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above in that six (6) children's files that were reviewed duing the inspection were incomplete which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that the licensee did not have documentation of the infants' 15 minute sleep log which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation, the licensee did not comply with the section cited above in that Child 2 and Child 6 were observed to be in a stroller when it was not used for transportation which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above. LPA observed a wooden safety gate and a plastic safety gate leading to the side areas of the homes to be broken and having sharp edges which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview, the licensee did not comply with the section cited above. Licensee was unable to provide documentation showing she has a current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview, the licensee did not comply with the section cited above. Licensee was unable to provide documentation for child #1 (C1) showing she is conducting and documenting infant safe sleep every 15 minutes during nap times which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA's observation, and interview 2 out of 2 play yards for infants had loose objects (sheet and blankets) which poses a potential risk to health and safety or personal rights to children in care.
