Food Service — Based on observation and interview, the licensee did not comply with the section cited above as snack menus have not been shared with parents since 10/2025 after changing communication software. A written menu for the following week is usually kept in the facility's kitchen on a dry-erase whiteboard, but it was not filled in during today's inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interviews with staff, the facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed the fire drill was conducted once a year .
Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed two staff member did not have LIC 503 (health screening). Director Stated that it was completed but does not know what happen to it.
