Upon entry at 11:45 am, LPA observed a total of six children in care alone with licensee. Four out of six children were observed to be infants which poses an immediate health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on interview, the licensee did not comply with the section cited above. Licensee was unable to provide proof showing fire and disaster drills are being conducted and documented at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. LPA observed toys hanging and attached to the side of the crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. Licensee was unable to provide proof showing a current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.

While reviewing the child care facility roster, LPA observed five children present were not listed on the roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee was unable to provide to LIC 9227 Individual Sleeping Plan form for infants child #1 (C1) and child #3 (C3) which poses/posed a potential health, safety or personal rights risk to persons in care.
