Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. At 1:52PM and 3:27PM. LPA observed that children went to the restroom. There was no staff standing where they can visually supervise the children in the restroom.
