During file review, LPA observed that assistant (S2) is missing the Mandated Reporter Training.
Staff immunization — influenza, pertussis and measles — Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During file review, LPA observed that assistant (S2) is missing proof of tuberculosis test and immunizations for measles, pertussis and influenza/statement declining influenza.

During file review LPA observed that three children (C3, C4 and C5) are missing the Consent for Emergency Medical Treatment (LIC 627) and signature on other forms.
