Fixtures, Furniture, Equipment and Supplies — Based on interviews and records reviewed, C1 was not instructed to utilize the River stones in an appropriate manner by utilizing thier hands when crawling or climbing. C1 was running and tripped and the floor that was not cushioned, causing C1 to hit their forehead.
