Personal Rights — Based on LPA observations and interview with pertinent individuals the licensee did not provide a safe environment. During interview inspection, LPA observed many small toys on the facility floor where infants crawl. This is a potential risk to the health and safety of children in care.
Staffing Ratio and Capacity — Based on LPA's interviews with pertinent individuals the licensee did not comply with the capacity requirement of a Large Family Child Care Home and was over capacity for an entire day. This is a potential risk to the health and safety of children in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that they did not have a sleep log available for the department to review which posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in that the licensee could not provide proof of MMR and TB for live in fiancee/assistant which posed a potential health, safety or personal rights risk to persons in care.
