Buildings and Grounds — Child 1 was startled and cried during the nap time when bookshelf tipped, and toys fell on the child .Child 2 hit their head on the glass table getting deep cut on the forehead.
Personal Rights — Per video footage provided to LPA on 02/13/25, S3 pulled the child from the shoulder and forced them to sit in rough manner, which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — S2 records were reviewed and observed 12 ECE units were not issued from approved or accredited school, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observations, interview and records review ,the licensee did not comply with the section cited above. During the file review, LPA observed S1 was missing pertussis and S5 was missing pertusis and measles,which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on observation, interview and record review, the licensee did not comply with the section cited above in S2 did not have Statement Acknowledging Requirement to Report Child Abuse (LIC 9108) and Employee Rights (LIC 9051), which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on observation, interview, record review, the licensee did not comply with the section cited above in 3 of out 6 children were missing physician’s reports in file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on observation, interview, record review, the licensee did not comply with the section cited above 1 out 6 children were missing immunizations record for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on observation, interview, record review, the licensee did not comply with the section cited above in 3 of out 6 children were missing Parent’s rights (LIC 995) and 3 of out 6 children were missing Acknowledgment of Receipt of Licensing Reports (LIC 9224) for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observations ,interview and records review the licensee did not comply with the section cited above , during the file review staff #1 were misiing health screening report , staff #2 was missing proof of TB test , staff # 5 was missing proof of MMR , which poses/posed a potential health, safety or personal rights risk to persons in care.
