H&S 1596.8595(a)(1) Each licensed child day care facility shall post a copy of any licensing report pertaining to the facility that documents either a facility visit or a complaint investigation that results in a citation for a violation that, if not corrected, will create a direct and immediate risk to the health, safety, or personal rights of children in care. The licensing report provided by the department shall be posted immediately upon receipt, adjacent to the postings required pursuant to Section 1596.817 and on, or immediately adjacent to, the interior side of the main door to the facility and shall remain posted for 30 consecutive days. This requirement has not been met as facility did not post the report or the Notice of Site visit from the inspection dated 07/01/2026. This poses an immediate risk to the health, safety and personal rights of children in care.

101430 Infant Care Activities (a) Notwithstanding Section 101230, the following shall apply: (3) All infants shall be given the opportunity to sleep without distraction or disturbance from other activities at the center whenever the infant desires. (C) An infant shall not be swaddled while in care. S1 was observed swaddling C2 and placing C2 in a crib to sleep. This poses an immediate risk to the health, safety and personal rights of children in care.

Notice to parents that site visit reports are available — Facility has not provided a copy of the report dated 07/01/2026 to parents or authorized representatives of currently enrolled children. This poses a potential risk to the health, safety and personal rights of children in care.

Personal Rights — This requirement has not been met as evidenced by: S1 force fed C1 a bottle. This poses an immediate threat to the health, safety and personal rights of children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 3 out of 6 persons did not have an update mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Activities — Based on record review, the licensee did not comply with the section cited above in 4 out of 5 which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 5 out of 6 persons did not have flu vaccination/declination and 1/6 staff did not have immunization records in their file which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 0 out of 6 persons did not have an update mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Modifications to Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 files infant's needs and services plan was expired, which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — LPA's record reviews did not observe the facility maintain updated immunization records for any infant in care immunization records due to receive required immunizations after enrolled. This posed an potential risk to the health and safety of children in care.

Daily Inspection for Illness — LPA's interviews with staff and LPA's own observation that the facility did not perform daily inspection for illness during the children drop-off as required by the regulations. This posed an potential risk to the health and safety of children in care.
