Infant Safe Sleep — Based on observation, interview, and record review, the licensee did not comply with the section cited above.LPA reviewed 2 infant's file and did not see the LIC 9227. Licensee stated she knows about the form but did not have parents complete the form which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview, and record review, the licensee did not comply with the section cited above.Licensee's assistant stated she does the sleep checks and logs them every 15 minutes when the infant's are asleep and every Friday she takes the logs home and hasn't done them in 3 days which poses a potential health, safety or personal rights risk to persons in care.
