At 1:00 p.m., during document reviews and interviews, both assistants stated that they didn't know where the fire drill papers were, which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — children supervised at all times — Based on observation, interview, and record review. The licensee did not comply with the section cited above. Around 1:53 p.m., another assistant (S2) arrived. Both Assistants stated the Licensee is currently out of the country and had left on 2/1/2026, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview, and record review. The licensee did not comply with the section cited above. At 1:00 pm, during the records review and interviews, the assistant stated that she did not have LIC 9227 Individual Infant Sleeping Plan for Child #2 in the file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on, observation, interview, and record review. The licensee did not comply with the section cited above, During the records review, LPA did not see any record of licensee reporting to the department that they will be away from the facility for any period of time, which poses/posed a potential health, safety or personal rights risk to persons in care.
