Family home — home free of safety hazards — Based on record review and interview at 1:45pm, L1 has not conducted a fire drill in the last 6 months. Last drill was conducted 01/07/2025, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, L1's Mandated Reporter training expired 09/2025 and assistant S1's Mandated Reporter training expired 10/2024. Based on interview with L1 at 2:00pm, L1 and S1 have not updated their training since the certificates on file were issued, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview and record review, Licensee stated at 12:13pm that he did not have current sleep logs for infants C1 & C2 and must have lost them. A review of records indicated that the last sleep on file was dated 02/22/24 for C1 and 03/06/24 for C2. The licensee did not comply with the section cited above for two out of two infants present during the visit, which poses a potential health, safety or personal rights risk to persons in care.
Family home — children supervised at all times — Based on interview with S1, the licensee did not comply with the section cited above. Assistants S1 and S2 stated that Licensee is on vacation and has not been present today and will not be present during the hours of operation today, which poses a potential health, safety, or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, at 10:25am LPA observed infant C6 sleeping in a play yard covered with a blanket.
Infant Safe Sleep — Based on observation, at 1:09pm LPA observed infant C7 sleeping in a play yard, inside of the play yard, C7 slept on a loose play mat with a toy bar arched over head, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation,beginning at 1:08pm LPA observed infant C1 asleep in an infant swing in the living room and C3 was observed sleeping in infant swing in the bedroom, which poses a potential health, safety or personal rights risk to persons in care.
