Infant Safe Sleep — Based on observation, interview, and record review, the licensee did not comply with the section cited above, which poses an immediate health, safety, or personal rights risk in persons in care. LPA observed one infant (C1) swaddled and placed on a raised cushion and restricted by velcro while sleeping inside a crib.
Staffing Ratio and Capacity — Record review, specifically attendance registers submitted by licensee for payment revealed, licensee provided care for more than fourteen (14) children at a time during the months of June 2023-November 2023. This is an immediate risk to the health and safety of children in care.
Staffing Ratio and Capacity — Based on LPAs observations and record review it was revealed Licensee was out of ratio from 7:45am until 7:56am. During this time, FCCH had 7 children of which 3 were infant age which is an immediate risk to the health, safety or personal rights risk to persons in care.

Current roster of children — Based on interview and record review, the licensee did not comply with the section cited above, which poses an potential health, safety, or personal rights risk in persons in care. Licensee stated she did not have all children listed on the roster, which LPA reviewed to be incomplete.
Child's Records — Children record review revealed C1, C2, C3, C7, C8, C9, C11, C13, C14, C15 and C16 had incomplete child records for review in the facility. In addition, C4, C5 and C6 did not have a file for review during inspection which poses/posed an potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review C9 and C14 did not have LIC 9227 for review which poses/posed an potential health, safety or personal rights risk to persons in care.
