Immunizations — Based on observations , records review and interview , the licensee did not comply with the section cited above. During the review , LPA observed 2 out of 10 children`s files were missing immunizations , which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on licensee's file review, the licensee did not comply with the section cited above and licensee did not have current mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on staff file review, the licensee did not comply with the section cited above and licensee's assistants, Rita Keshishyan and Satenik Yeritsyan did not have proof of immunization against Pertussis and Influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.
