Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in LPA observed children wood table, playhouse chipping in the play yard. LPA also observed a dirty chair base with no cover which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, the licensee did not comply with the section cited above in LPA observed Staff #2 (S2), S3, S4 do not have a completed Mandated Reporter Training AB1207 which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation, the licensee did not comply with the section cited above in LPA observed Staff #1 (S1) and S4 do not have proof of influenza vaccine or declination letter. S6 does not have proof of MMR and Tdap vaccine which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observation, the licensee did not comply with the section cited above in LPA observed none of the staff had an approved Pediatric CPR/FA. DIrector had a Pediatric CPR/FA from National CPR Foundation ex: 09/09/2025 (Not EMSA approved) which poses/posed a potential health, safety or personal rights risk to persons in care.
