Staff immunization — influenza, pertussis and measles — Based on record review and interview, the licensee did not comply with the section cited above when the Licensee stated S1 has not been immunized against measles and pertussis. S1 file did not contain evidence that S1 had received the immunization's which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in six out of six child files did not contain the PM 286 as required, which poses/posed a potential health, safety or personal rights risk to persons in care.
Operation of a Family Child Care Home — Based on record review, the licensee did not comply with the section cited above in six out of six child files did not contain an affidavit regarding informing the parents that the family child care does not carry liability insurance, which poses/posed a potential health, safety or personal rights risk to persons in care.
