Child Care Center Director Qualifications and Duties — On 03/18/2026, LPA reviewed S1 file and S1 did not have the health and safety training certification required to be a director. S1 stated they have not completed the required training as of 03/18/2026.
Health-Related Services — During the visit on 03/18/2026, S1 stated there were children enrolled with medication. LPA observed three (3) out of nine (9) children medication had been expired.
101229(a)(1) Responsibility for Providing Care and Supervision: (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time…. Supervision shall include visual observation. Based on interviews and record review, the facility did not comply with the section cited above. 5 out of 5 staff confirmed that Child 1 (C1) wandered away from the classroom into enclosed playground due to lack of supervision for about two minutes which poses a potential risk to the health, safety or personal rights risk to persons in care.

101212(d)(1)(C) Reporting Requirements (d) Upon the occurrence, during the operation of the child care center…a report shall be made to the Department by telephone… In addition, a written report shall be submitted to the Department within 10 working days (1) Events report shall include… (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Based on LPA's record reviews and interviews, five (5) interviewed staff confirmed of incident in which C1 wandered away from the classroom without adult’s supervision, and there was record of the incident being reported to C1’s representatives, however, facility did not report the incident to the Department, which posed a potential risk to the health, safety and personal rights to the children in care.
