At the time of the inspection, LPA observed 4 months old baby in a rocker seat (bouncer) on the floor. LPA had discussed these banned items with the licensee on the last pre-licensing inspection.

At the time of the inspection, LPA observed 4 pack and play in the bedroom with blankets, pillows and hanging items from one of the pack and play.
Child's Records — Based on observation and record review, the licensee did not comply with the section cited above in 2 counts out of 5 files missing LIC 700 form which poses a potential health, safety risk to persons in care. Child # 1 and # 3 missing LIC 700 and LIC 700 for child # 2 was incomplete.
