The licensee stated that on the morning of February 4, 2026, a car crashed into the front door of the neighboring business. The licensee did not report the incident to Licensing within 24 hours of its occurrence, which poses a potential health, safety, or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on information obtained through record review, it was revealed that the facility had been out of ratio. This poses a potential risk to the health, safety, and personal rights of children in care.
