Immunizations — Based on Licensee statement, a child (C9) has been attending since April 2026 without complete record of their immunizations which poses a potential health, safety, or personal rights risk to children in care.
Buildings and Grounds — Based on LPA observation and staff interview, a potty chair was present outside on the playground while children were present in an area accesible to children. After two children used the potty, LPA observed no staff immediately remove the potty to clean it. During interviews, staff 1, 2, 3, and 5 stated that the potty is used when there are not enough staff in ratio to bring children inside to the bathroom.
Teacher Aide Qualifications and Duties — Based on LPA observation and file review, staff 5 (S5) was supervising 12 children in the art room by herself and did not possess teacher qualifications.
Teacher-Child Ratio — Based on LPA observation and file review, staff 5 (S5) was supervising 12 children in the art room by herself and did not possess teacher qualifications.
Mandated reporter training — Based on record review, staff 2 (S2) and staff 4 (S4) did not possess current mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
