LPAs observed spider webs in various play areas, loose wooden fence boards leaned up against the fence to cover broken wooden fence boards of the original fence, dog feces and a hole in the dirt with a large black plastic pipe exposed which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in 5 out of 5 children did not have immunizations on file which poses a potential health, safety or personal rights risk to persons in care.
