Sign In and Sign Out — Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed that some authorized representatives had not signed children in and out. LPA reminded Licensee Representative that authorized representatives must sign children in and out at drop off and pick up times.
Responsibility for Providing Care and Supervision — staff did leave day care children unattended in the Orange Room on December 12, 2023. This presents a potential risk to the health, safety, or personal rights of children in care.
Buildings and Grounds — Facility did have ants in the kitchen area of the Facility back in early January 2024. This presents a potential risk to the health, safety, or personal rights of children in care.
