Buildings and Grounds — Based on interview and record review, the center had a power outage on 07/24/2024. Staff interviews verifying the temperature had exceeded 85 degrees Fahrenheit and facility records showing logs of temperatures above 85 degrees Fahrenheit on 07/24/2024. This poses a potential health and safety risk for children in care.
False Claims. No licensee, officer or employee of a licensee shall make or disseminate any false or misleading statement regarding the child care center or any of the services provided by the center. This requirements is not met as evidenced by: Based on interview, LPA Yip was informed by Regional Manager that the temperature of the rooms had reached above 85 degree Fahrenheit at 1:30PM, however, one of the class at reached above 85 prior to 1:30PM. This poses a potential health and safety risk to children in care.
Responsibility for Providing Care and Supervision — Based on observation, LPA observed at 11:20 that there were children in the toddler play yard. There was no staff standing where they can visually supervise the children. Director brought the child to the staff. This poses a potential health and safety risk to children in care.
Criminal Record Clearance — Based on observation, interview, and record review, A-1 is left alone with children. A-1's fingerprints were disassocaited and not transferred back to facility. A-2 was also present at the facility and has cleared fingeprints, but fingerprints were not associated to facility. Both A-1 and A-2 are associated to other Primary Plus facility roster. This poses a potential health and safety risk to children in care.
Responsibility for Providing Care and Supervision — Based on observation, LPAs observed at 11:40AM that a staff walked out of the bathroom while a child was inside. There was no staff standing where they could visually supervise the child. Staff were in the room. This poses a potential health and safety risk to children in care.
