Personal Rights — Interviews disclosures Staff #10 yells at children while in care. Although, staff #10's interaction are limited with children, when Staff #10 does interact Staff #10 is heard and observed to be negative raising her voice, scolding children, speaking to children in an inappropriate manner. Interviews disclosed staff #10 yells at children. Staff #10 also was witnessed yelling at staff and parents while children are present. This poses a potential health and safety health risk to children in care.
101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met by... Evidence by interviews and record review. Staff #1 was observed mishandled child #1 by holding child #1 down on the cot. This poses a potential Health and safety risk to children in care.
Personal Rights — Based on interviews and photo the licensee did not comply with the section cited above in staff not providing adequate supervision resulting in day care child sustaining injuries to the left eye which poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above. LPA observed a ceiling panel not placed properly and at risk of falling off which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. LPA observed toilets in the preschool bathroom to be unclean which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation , the licensee did not comply with the section cited above. LPA observed outdoor metal planter rusted with pieces peeling/ coming off which poses/posed a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation the licensee did not comply with the section cited above. LPA obseved a dark substance in the back wall of the refrigerator which poses/posed a potential health, safety or personal rights risk to persons in care.
