Buildings and Grounds — both staff bathrooms were not in good repair, which posed a potential risk to the health and safety of the employees present.
Immunizations — Four (4) of the children's files reviews showed out of date immunization records. Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Modifications to Infant Needs and Services Plan — Twenty-nine (29) Infant Needs & Services Plans reviewed have not been updated quarterly. Based onrecord review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Aide Qualifications and Duties — A teachers aide was left alone by their co-teacher with children on multiple occassions which poses a potential risk to the health and saftey of the children in care.
Personal Rights — Two children in care reported that a staff member had pinched them on seperate occassions which is a potential risk to the health, safety and personal rights of the children in care.
Mandated reporter training — through record review it was found that 5 staff members do not have a valid mandated reporter training. This poses a potential risk to the health, safety & personal rights of the children in care.
Criminal Record Clearance — One staff member was present providing care and supervision without haveing a criminal record clearance. This poses a potential risk to the health, safety, and personal rights to the children in care.
