Written directives for drinking water lead testing — Based on records review of required lead testing, the facility had lead values of 5 ppb or above on water outlets: outlet C 7.4 ppb, outlet D 10 ppb, and outlet E 13 ppb, on testing dat 1/24/26. This poses/posed a potential health, safety, or personal rights risk to persons in care.
Reporting Requirements — Based on the observation/interview/record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arrival the LPA was greeted by S4 who introduced themself as the Site Supervisor. The LPA was informed the Site Supervisor on file has not been the Site Supervisor for a month and S4 accepted the position.
Limitations on Capacity and Ambulatory Status — Based on information obtained, the Licensee did not operate the facility according to the License. The facility was providing care in Room #31 which was not approved by the department. This poses a potential health & safety risk to persons in care. **This is an amended report**
